Complete Blood Count Explained: Haemoglobin, White Cells and Platelets

Every year, millions of people receive a slip of paper covered in numbers, abbreviations, and reference ranges that might as well be written in another language. That slip is the result of a Complete Blood Count — one of the most routinely ordered and diagnostically powerful tests in modern medicine. Far from being a routine formality, this test offers a remarkable window into your body’s internal workings, revealing clues about everything from infection and anemia to bone marrow health.

The Oxygen Carriers: Understanding Haemoglobin

At the heart of the CBC lies haemoglobin, the iron-rich protein housed within red blood cells that is responsible for transporting oxygen from the lungs to every tissue in the body. When hemoglobin levels fall below the normal range—typically 120–160 g/L in adults, though this varies by sex and age—the condition is known as anemia. Symptoms can range from subtle fatigue and breathlessness on exertion to more severe manifestations like rapid heartbeat and cognitive fog.

But low hemoglobin tells only part of the story. Clinicians also examine the size and color of red blood cells—measured through values like MCV (mean corpuscular volume) and MCHC (mean corpuscular hemoglobin concentration). A low MCV alongside low hemoglobin often points toward iron deficiency, whereas abnormally large red cells may indicate a deficiency in vitamin B12 or folate. This layered interpretation transforms a single number into a diagnostic narrative.

The Body’s Defence Force: Decoding White Cell Counts

White blood cells are the immune system’s frontline soldiers, and the CBC breaks them down into five distinct subtypes: neutrophils, lymphocytes, monocytes, eosinophils, and basophils. This breakdown, called a differential count, is where much of the diagnostic richness lies.

An elevated total white cell count—leukocytosis—frequently signals a bacterial infection or inflammatory response. However, context matters enormously. A surge in neutrophils specifically suggests acute bacterial infection, while a rise in lymphocytes may indicate a viral illness or, in some cases, a lymphoproliferative disorder. Conversely, a dangerously low white cell count (leukopenia) can leave the body alarmingly vulnerable to infection and may be a side effect of chemotherapy, autoimmune disease, or bone marrow suppression.

Clinicians never interpret these figures in isolation. Alongside symptoms, physical examination findings, and other laboratory results, the white cell differential helps build a coherent clinical picture.

Tiny but Critical: The Role of Platelets

Platelets are the smallest formed elements in the blood, yet their importance is outsized. These cell fragments are essential for hemostasis—the process by which bleeding is stopped after injury. They aggregate at sites of vascular damage, forming the initial plug that prevents excessive blood loss.

A platelet count below 150,000 per microliter (thrombocytopenia) can increase the risk of bruising and prolonged bleeding, while counts above 450,000 (thrombocytosis) may raise the risk of abnormal clotting. Both extremes warrant investigation, as causes range from nutritional deficiencies and viral illness to more serious hematological conditions.

Understanding your CBC results empowers you to have more meaningful conversations with your healthcare provider. Rather than passively receiving a “normal” or “abnormal” verdict, ask about trends over time, request an explanation of any flagged values, and ensure follow-up testing is arranged when warranted.
